Services
Patients who come to our facility have many different recovery support needs- some are coming right out of a residential treatment program, while others admit to Foundations Atlanta as an entry-level treatment for their addiction, before it consumes their life. Those suffering with chronic pain and addiction may come to Foundations Atlanta to participate in our compassionate and effective treatment program exclusively designed to address their unique needs. Whatever the problem may be, services available at Foundations Atlanta are designed to accommodate each patient's individual needs. While providing the structure of a treatment program- with the freedom of the outside world- we are able to continue to guide patients in their recovery each step of the way.
To achieve a successful recovery, our services are built around the individual's real life so that the patient can continue to spend time with their families, work and/or attend school while also getting help for their addiction(s). These services include Care Management Programming, Relief and Recovery Services (chronic pain with addiction), Professional Assessments, Monitoring and Aftercare Program, Alternative Sentencing (Pre-trial Intervention) and Substance Use Disorder Assessments.
Care Management
Sobriety can often be achieved through outpatient treatment when someone seeks help in the early stages of addiction. Additionally, outpatient services are critical components for long-term success in patients transitioning from residential treatment. Our goal is to serve those stepping down from higher intensity services as well as clients from the community at large.
Foundations Atlanta's Care Management programming is broken down into three TIERS of care based on your treatment needs and can be modified as clinically appropriate. TIER III is our highest level of care and the level of care that most patients are appropriate for when they first admit to Foundations. As their treatment progresses, patients are moved through lower TIERS of care to slowly integrate them back into the "real world."

Group Therapy: the backbone of each TIER is group therapy, where patients participate in education lectures, specialty focus groups and general process groups

Care Management Sessions: patients are offered one on one sessions with their primary therapist that can be structured as individual therapy or family therapy.
Multi-Family Group: once weekly, patient famililies are able to meet together for a group facilitated by a family therapist. Once montly, a Family Education Night is offered where families participate in education lectures and other specialty groups.
Holistic Interventions: part of a healthy recovery is re-learning self-care and other body mindfulness activities. As part of treatment, patients are able to choose a variety of holistic interventions such as massage, yoga and personal training.
