Our Three-Step Care Management Process

  • The Assessment
    You or your loved one will meet with one of our addiction specialists for an in-depth initial assessment. During this, you and your Care Management Team will determine what you want, what you need and how to get there. We will assess your place in the addiction/recovery process, physical health, mental health, spiritual needs, financial needs and many other issues. At this time you will be assigned a Care Manager and your assessment will determine what course of treatment best suits your needs.

    After your assessment and based on your individual needs, we will assemble your multi-disciplinary team of professionals that can include a psychiatrist, therapist/care manager and other professionals involved in treatment.


  • The Plan
    Together, you and your Care Management Team will put together a flexible plan created just for you that can change as you advance through recovery. The plan is based on the needs and desires of the client and focuses on their integration into the local community, family and natural support systems. If you join us following a residential episode, we will incorporate your continuing care plan that was developed with your previous treatment team, and we will communicate and work with them at your direction.


  • The Life
    We work together, as often as necessary, wherever necessary to implement the plan created. We join you in real-life settings, not in protective bubbles, and make changes based on your own process for recovery.

Our Three Tiers

Treatment planning at FRN is a dynamic process that involves ongoing modification to support growth and changes in the client. A key method for maintaining an action-oriented treatment program is to establish goals that are specific, important to the client and realistic to their needs.

Our Care Management Program consists of three Tiers varying in their level of intensity, based on the individual treatment needs and are modified as clinically appropriate. These three Tiers are as follows:
  • Level I- for the individual following a solid base of treatment or recovery who needs basic continuing care or "refresher" support after a recent lapse
  • Level II- for the individual following a residential treatment episode but who has a history of relapse and prior treatment
  • Level III- for the individual attempting to avoid residential long-term treatment or individuals following residential treatment who have a chronic history of relapse or multiple contributing/complicating factors placing the individual at high risk for relapse

Each tier includes:
  • Initial Comprehensive Evaluation
  • Random drug screening
  • Psychiatric visits and medication management
  • Weekly group treatment
  • Weekly Care Management sessions (Individual, family and couple's sessions)
  • Massage or other holistic intervention
  • Yoga/Pilates or other physical activities
By assessing which Tier is most appropriate, our team is able to place your loved one in the right level of care needed. Our goal is to create an individual program that will meet each unique need of the people we serve.
