The ABC's of Care Management

What is it, who is it for and how does it work?

Going over care management planWhat if standard residential treatment just isn't for you? Or what if you've gone through treatment programs in the past and have relapsed? Do you need to start from square one? Maybe it's not physically possible for you to leave home to get treatment. Are there options for you?

The answer to many of these questions is Care Management.

"Care management is appropriate for those who've been to residential care in the past, are just getting out of a residential program or someone who isn't able or willing to go to a residential program," explains Foundations Recovery Network's Dan Cronin.

So what makes someone with a substance abuse problem or in recovery unable to go to treatment or to more conventional aftercare programs? There are numerous reasons and situations. Here are just a few of the most common scenarios:

Past Treatment – "People that have had multiple treatment experiences and have not been successful sometimes get greater benefit and achieve their goals through a more individualized treatment plan in a real-life setting," Cronin says. While experts agree that residential treatment is the most effective overall, after multiple attempts the likelihood that it will be effective is lowered. It's the recovery world's version of the law of diminishing returns. There are people who get to the point where they know what to expect and, as a result, they don't get anything out of it. "There are people who could write that Zagat's guide to treatment centers, but they can't stay sober," Cronin says, adding, "They're not going to go to group therapy, but they'll go to individual therapy."

Leaving Treatment – So you've completed treatment and then returned home. Now what do you do for aftercare? For everyone from corporate executives and pro athletes to musicians and stay-at-home moms, traditional aftercare isn't always a good fit. With pro athletes and entertainers, their travel schedule means they're never in the same city, while execs have such demanding schedules they can't show up every day at 7pm. Then there are the moms charged with caring for young children who don't fit into the outpatient setting. So with this group in mind, Cronin decided to take the individualized treatment model and take it to the next level.

Care Management programs are tailored to each specific individual with the overall goal of long-term sobriety and success. That can range from support for getting through the first year to help spotting the signs of a potential relapse to guidance on getting to the next level. All of this is done in an atmosphere of setting manageable goals and celebrating achievements.
